Assistive Technology Funding Under the AT-HM Scheme

If you or someone you love receives aged care support at home, there have been important changes to how assistive technology may be funded.

On 1 November 2025, Australia’s Support at Home program replaced the Home Care Packages Program and the Short-Term Restorative Care Programme. Support at Home is designed to help older people keep living safely and independently at home for longer.

Here is what you need to know, and how Harmonie fits in.

What is the Support at Home program?

Support at Home is Australia’s new national in-home aged care program.

It replaced the former Home Care Packages Program levels with a new eight-tier classification system, offering more flexible, quarterly budgets tailored to individual care needs.

If you were previously on a Home Care Package, you will be transitioned to Support at Home. Your funding levels also may change from your previous package tier, so we recommend speaking to My Aged Care to confirm this.

What is the Assistive Technology and Home Modifications scheme?

This is one of the most significant improvements in the new program for families managing care at home.

Under the old Commonwealth Home Support Package system, if you needed assistive technology, such as a device to help manage medications or daily routines, you had to save up funding from your general care budget to pay for it. That meant waiting, and often going without.

Under the new Support at Home program, assistive technology and home modifications are funded separately, through the dedicated AT-HM scheme. You do not need to save from your ongoing care budget to access it.

The AT-HM scheme provides eligible participants with upfront funding specifically for assistive technology items and home modifications that help them live safely and independently at home. Funding is allocated across three tiers based on your assessed needs: Low (up to $500), Medium (up to $2,000), and High (up to $15,000). Higher amounts may be approved where there is supporting evidence.

What kinds of assistive technology are covered?

The AT-HM scheme is designed to fund devices and equipment that help you manage daily life at home more safely and independently. This includes things like:

– Mobility aids and walking frames
– Communication and information devices
– Medication management tools and reminders
– Daily living aids and adaptive equipment
– Home monitoring technology

The key test is whether the technology helps you do something more safely or more independently at home. If it does, it is likely to be considered eligible, though your assessor will make the final determination based on your individual needs and circumstances.

Can Harmonie be funded through the AT-HM scheme?

Harmonie is designed specifically to help people living at home stay organised, manage their medications and daily routines, and maintain independence, which aligns directly with the goals of the AT-HM scheme.

Whether Harmonie is suitable for funding through Support at Home will depend on your individual assessed needs and circumstances. We recommend speaking with your aged care assessor or occupational therapist about whether Harmonie would be appropriate as part of your care plan.

How do you access AT-HM funding?

You will be assessed for AT-HM funding as part of your aged care assessment through My Aged Care. If you are already a Support at Home participant, speak with your provider or care coordinator about whether an AT-HM assessment is appropriate for your situation.

Your assessor may recommend an occupational therapist to help identify which assistive technology best meets your needs. An OT recommendation can also strengthen your case for funding approval.

What if I am not on Support at Home?

If you are not currently receiving government funded aged care,
assistive technology may still be accessible through other pathways, including NDIS assistive technology funding if you are eligible, or direct purchase.
